Shot 480 rounds of mixed carry ammo (Federal 147 HST, Speer Gold Dot 115, 124, 124+p) and training ammo (mix of American Eagle 115, 124, and Syntech match ammo). Zero malfunctions. I have also tried it with the Beretta 80X Cheetah using the same PMC ammo I had in the .380 Sig. Again, no problems. That wasn’t really a surprise. The flat faces forward in the closed position. The rear end of the guide rod rests on the takedown lever (or pin). This requires the guide rod to protrude at the muzzle by about 3/16" or the difference between flat up and flat forward. Just a bad design on the 365, Ayoob's review of the 365 talked about this and he suggested using the left hand or moving the release to the left side.

Overall, the P365 ran great. I can't believe how soft it shoots. I did have some mag problems. I had some failures to feed with the Winchester Train 147gr fmj. The round stuck in the mag and did not reach the feed ramp. A single pull on the slide made the round feed normally. I did not have this problem with the three other brands of ammo.
